On commencement of care with Community Careline Services.
You will be given an information pack providing you with
information about our company. This will include relevant policies
and procedures, our complaints procedure, home fire safety
information and our care charges.
A care manager or care co-ordinator will visit your home to assess your care-package, develop your person centred care plan as according to your needs and wishes, and complete the appropriate care forms in order to comply with current legislation.
These forms are designed to provide your support worker with the relevant information to enable them to deliver a service to meet with your needs and requirements. We only ask that you keep your care plan and care forms in a safe, private place accessible only, to your support workers and individuals of your choice.
